By focusing on a person’s skills, gifts, and abilities these person-centered planning services help people with other abilities achieve their goals and live a life based on their vision.  

We include PCP’s, spending plans, regional center collaboration and a warm hand-off to ensure a successful transition into The Self Determination Program.
